The only requirement I had was being enrolled with a center of excellence before starting coverage.
I have UHC insurance with a fertility benefit of a $15,000 lifetime limit. Through my online portal, I can view what is covered and not covered. Do you have an online account with UMR? Make sure to check your benefits to see what is included. In order to be covered, a diagnosis of infertility and enrollment in “fertility solutions” is required. Some plans may have specific requirements for coverage, such as a certain number of IUI’s before IVF. My clinic obtained prior authorization before proceeding with IVF, which I appreciated. It may be beneficial to find a clinic that can do the same if you are just starting out. Your first step should be finding a clinic if you don’t have one yet.
