As a former nursing supervisor at a clinic, I would be concerned about the number of errors made by my staff. Ensuring patients take their medication correctly is already challenging, so providing incorrect instructions multiple times could lead to the clinic having to cover a cycle of meds. The nurse could be covering for a colleague, lacking proper training, or facing limitations in the system or communication from the MD. Troubleshooting is necessary to address this issue.
